Psychosocial impact of malocclusion in adolescents from schools in Asunción and Pirayú, Paraguay

Introduction: Self-esteem and well-being may be affected due to oral problems. The malocclusion occupies the third place of prevalence in the world.   Objective: To determine the prevalence of the need for orthodontic treatment and to relate it to the psychosocial impact in adolescents aged 12 to 16 enrolled in public schools in Asunción and Pirayú (Paraguay) in 2017.   Method: Descriptive observational cross-sectional design. The need for orthodontic treatment was measured using the Dental Aesthetics Index (IED), using the periodontal probe PCPUNC15, and the psychosocial impact with the Dental Aesthetics Psychosocial Impact Questionnaire (PIDAQ). 206 adolescents participated in the study. The schools were selected for convenience. Pearson Chi-square tests of homogeneity, Mann-Whitney U and Kruskal Wallis U were applied with a confidence level of 95%.   Results: 61.7% were women, 38.3% from Asunción and 61.7% from Pirayú. The need for malocclusion treatment was 19.0% in Asunción and 4.0% in Pirayú, the difference being significant (p = 0.001). The mean PIDAQ was 58.22 (SD = 9.97) in Asunción and 61.77 (SD = 14.87) in Pirayú, a significant difference (p <0.001). The sex distribution was homogeneous for FDI and PIDAQ. The “confidence in one’s own dental aesthetic” subscale differed significantly according to the need for treatment (p = 0.034).   Conclusions: the adolescents in this study, who attend rural schools, presented less need for orthodontic treatment and better perception of their dental aesthetic aspect than those of urban schools. It is necessary to go deeper into the risk factors involved.
