The incidence of a second palatal root in the maxillary second molar is a very unusual anatomic variation. This case report describes a treatment of a fourfold root maxillary second molar with two distinct palatal roots. A clinical examination and radiographs showed the presence of a second palatal root with its own canal. After biomechanical preparation with ProTaper, a rotary file system, root canals were filled with ProTaper gutta-percha cones and number 25 accessory cones and TopSeal, a resin based cement. After treatment tooth was sealed with temporary cement and it’s referred for a final restoration.
